To apply for enrollment in a Collaborative Licensure Program, please download the Licensure Program Application Packet and follow the steps on the Application Checklist.

Our Licensure staff are available to discuss program and admissions requirements with each candidate. We review state regulations governing licensure, options for completing the licensure program only or combining licensure with completion of a Master’s Degree program.

Application Checklist
Every applicant must fully complete, sign, and date these forms:
- (Required) Enrollment Application Form-A (includes your Licensure Program selection)
- (Required) Commitment Letter Form-B (includes your payment option selection)
Include with your application:
- Letter of Candidacy and/or essay describing your goals
- Non-refundable $50 Application Fee, payable to The Collaborative for Educational Services
- Resume
- Official transcript(s) of Bachelor’s Degree(s)
- Evidence of any graduate degrees, if applicable
- Copies of Current Licenses/Certificates
- Test Results (Qualifying Scores) on MTEL tests as appropriate for the program
- Three (3) signed Letters of Reference, which are persuasive about:
- good moral character
- capacity to work effectively in an organization
- capacity to succeed in the current standards-based approach to education
- knowledge that will permit mastery of the Massachusetts Curriculum Frameworks
